Informing about '' poliovirus formulario '' by the Consulate General of Italy

The Consulate of Italy informs all the Visa applicant who resided for more than 4 weeks, starting from 1st of January 2017, in the following Countries:   Afghanistan,  Somalia,  Pakistan,  Nigeria, Siria, Democratic Republic of the Congo and Kenya, that is mandatory to submit, along with the rest of documentation, also a Polio Vaccination Card that states that the applicant has received a dose of vaccine within 4 weeks and 6 months prior to the period spent in the above mentioned Countries.
